All of the patients in this book are arranged in alphabetical order, just like an office. All the forms that you will need to code these services, such as CMS-1500, UB-92, and MedBooks' DOC forms are also included. While medical charts and records are being converted to the EMR (electronic medical record) format, physician handwriting has not changed. Medical coders, billers, and medical assistants must continue to decipher their handwriting.
