The purpose of this book is to address the needs of clinicians, teache rs, students, and clinical researchers by bringing together a synthesi s of the literature based on evidence to provide answers to some of th e many questions encountered on a daily basis in clinical practice. Su ch an approach can help identify important questions for further inves tigation and may also furnish us with a template to apply to future ch allenges in causation, diagnosis, and treatment as new evidence accumu lates.
