Developing Primary Care for Patients with Long Term Mental Illness

This work aims to show how primary and secondary health care groups can overcome their traditional independence and work togther to improve care for the long term mentally ill. To facilitate this, the book provides a framework for the decision making which is necessary to plan care tailored to the needs of local patients, practices, and community teams.
