The Patient Safety Assessment Manual is a component of the WHO Patient Safety Friendly Hospital Initiative. The manual integrates different standards that are directly related to the recommended WHO patient safety interventions and challenges. These standards cover five patient safety domains: leadership and management, patient and public involvement, safe evidence-based clinical practices, safe environment, and lifelong learning. The manual is intended to provide health care professionals with practical guidance on how to identify patient safety gaps at the hospital level. These gaps can then be addressed through an improvement program supported by a patient safety culture at the institutional level.
